* The Subscriber is the person who pays for the health plan. For example, if your child has a Healthfirst Child Health Plus plan and you pay for the premium, then you are the subscriber.
** Your Account Number is 15 digits (Leaf Plans and Essential Plans) or 6 digits (Child Health Plus), and is located on the top-right of your invoice. Your Marketplace ID is 12 characters which starts with "HX" followed by 10 numbers, it is available on your NYSOH Marketplace account under the eligibility tab.
